Participation Waiver Form

If you're interested in being an Honorary Captain at one of our participating schools Varsity High School games or one of our many sponsored events, please fill out our Waiver Form and send it back to us via email (info@aha-inc.com) or fax (888-270-8388).

Submitting a signed and dated Waiver Form is a requirement prior to taking part in these events. This waiver is good for the entire scholastic year.

L.I.N.C.S. - Council Rock Service Learning Program

The new L.I.N.C.S. form has been updated for the 2008-09 school year. It is a now a two-sided form, with the pre-approval on one side and the verification on the other. They are available in the Career Center, Counseling Office, and front reception desk. These forms may also be downloaded from our web site - please be sure to print both pages when doing so. We will continue to accept the old forms throughout this school year.
